Clinical Quality

Patients that lack adequate health insurance, low-income, reside in public housing or rural areas often cannot access preventive and primary care services and may not have a regular source of care.  As a result, health problems may go undiagnosed and/or untreated, increasing the long-term risk of serious illness and death among the medically underserved.

Electronic Health Records:

An Electronic Health Record (EHR) is an electronic version of a patient’s medical history maintained by their provider.  The EHR should include all of the key administrative and clinical data relevant to that patient’s care.  Information maintained in the HER may include demographics, progress notes, problems, medications, vital signs, past medical history, immunizations, laboratory data and radiology reports.

International Classification of Diseases

The International Classification of Diseases (ICD) is the standard diagnostic tool for epidemiology, health management and clinical purposes.  This includes the analysis of the general health situation of population groups.  It is used to monitor the incidence and prevalence of diseases and other health problems.
