Association of Health Literacy With Poor Diabetes Outcomes
The purpose of this session is to review the relationship between health literacy and health outcomes in patients with diabetes and discover what the potential interventions are to improve such outcomes.

Community Partnerships to Address the Consequences of the COVID-19 Pandemic Among Residents of Public Housing
The COVID-19 pandemic and its associated mitigation strategies are expected to have significant psychosocial, behavioral, socioeconomic, and health impacts, which are exacerbated in populations that experience health disparities and other vulnerable groups. Those experiencing health disparities prior to the COVID-19 pandemic are at increased risk of infection and other COVID-19 related consequences, such as job loss, unpaid leave, and lost wages. Given the urgent need for prevention and mitigation strategies (i.e., physical distancing, wearing face coverings, frequent handwashing, disinfecting surfaces, shelter-in-place, self-isolation upon suspected exposure, leaving home only for essential activities, etc.), there is also a need to leverage and scale existing partnerships, resources and platforms and ongoing mitigation efforts within communities to attenuate adverse outcomes. This learning collaborative explored existing or developing partnership strategies to address the negative health impacts of COVID-19 in vulnerable populations.

COVID-19 & Diabetes: Challenges & Opportunities to Help Vulnerable Populations 
In this learning collaborative session, the National Center for Health in Public Housing and the Health in Public Housing Clinical Quality Work Group discussed the latest challenges faced by PHPCs related to COVID-19 and diabetes care. Best practices and Telehealth concerns associated with health care service delivery were among other topics addressed in this call. 
 

COVID-19 Pandemic: Interventions and Challenges in Public Housing Primary Care 
During this call, NCHPH and PHPC Health Centers discussed some of the most pressing needs for PHPCs in response to COVID-19. Health Centers shared their best practices to prevent COVID-19 infections among health center staff and their current patient management mechanisms. Other topics of discussion also included HUD guidance, community collaborations, challenges and training and technical assistance needs. 

Culturally Competent Care Learning Collaborative
Increasingly diverse racial, ethnic, and socio-cultural backgrounds of patients, colleagues, and staff challenge physicians as they strive to deliver care to their patients. Cultural and language difference may engender misunderstanding, a lack of compliance, or other factors that negatively influence clinical situations and the health outcomes of our nation’s diverse populations. As a result, the Office of Minority Health (OMH) at the U.S. Department of Health and Human Services (HHS) developed national standards for providing culturally and linguistically appropriate services (CLAS).
To assist Health Centers in the adoption of the CLAS Standards and to equip providers with the competencies that will enable them to better treat the increasingly diverse population, NCHPH will be offering a 4-part learning collaborative. The interactive sessions will cover the fundamentals of cultural competency, language access, and building community partnerships through a combination of case studies and didactic learning.

Empowerment and Self-Management of Diabetes – The Pharmacist and Diabetes Care Learning Collaborative
The goal of this learning collaborative is to discuss with pharmacists how to integrate diabetes education and management into practice, so patients can make the best use of their medications and achieve the desired therapeutic outcomes.

Exploring Cultural Competence & Humility in the Care of HIV Patients
This training was designed to help move organizations past cultural stereotypes and biases (e.g., based on race, sexual orientation, gender identity) to offer client-centered services with cultural humility. During this session NCHPH collaborated with the Southern AIDS Coalition (SAC) to provide participants the opportunity to better understand identity and diversity as we explored the long history of identity in the context of serving populations living with and/or impacted by the HIV/AIDS epidemic. Focus points for this session were also to provide a brief overview of PHPC grantee patients living with HIV.

Health in Public Housing Clinical Quality Work Group Call: Increasing Access to Care
During this call, NCHPH and PHPC Health Centers discussed the latest challenges and best practices related to COVID-19 and access to health care services. Other points of discussion were TTA priorities and health center issues.

Improving Public Housing Health Center Service Delivery Through Cultural Competence and Health Literacy Learning Collaborative
Research on health literacy from the Institute of Medicine (IOM) states that over 90 million U.S. adults do not have needed literary skills to access and navigate U.S. health systems (Rudd & Anderson, 2006). As a result, patients disengage from the health system until they have no choice, thus ignoring their care and seeking alternative remedies or ways of coping.

The Impact of Nutrition on Diabetes Prevention and Diabetes Management
This presentation will cover best practices for nutrition care, top eating patterns for people with diabetes, and ADA resources that can be used in practice.

On this call, PHPC Health Centers discuss the changing face of public housing, strategies to increase access to care in public housing primary care, and current and emerging issues affecting Health Centers located in and/or immediately accessible to public housing developments.

Supporting Implementation of Smoking Cessation Programs in Public Housing Primary Care Settings 2020
This Learning Collaborative aimed to identify barriers to implementation of and strategies to establish smoking cessation programs in primary care settings through a series of four sessions: a didactic webinar, covering the behavioral aspects of cessation counseling, the FDA approved medications for cessation and some basic motivational interviewing techniques, and a subsequent three learning collaborative sessions, detailing how to conduct each counseling session whether they are individual or group. NCHPH provided a written protocol booklet to be used as a guide during actual sessions.

The Use of CHWs in the Prevention of Teen Pregnancy and HPV Vaccine Promotion in Hispanic Adolescent Girls Living in Public Housing
Teen pregnancy and HPV vaccination continue to be a pressing issue for low-income populations. 
Human papillomavirus (HPV) is a common sexually transmitted infection which is the cause of several cancers, including cervical cancer, and genital warts. Although cervical cancer can be prevented through screening, this cancer persists in the US. More recently, HPV vaccination has the potential to decrease the burden of HPV-related disease among young HPV-unexposed adolescents.  This learning collaborative will explore the role of community health workers in teen pregnancy prevention and HPV vaccine promotion in teenage girls living in public housing.
