People are shaped by their culture. An individual’s diverse beliefs, behaviors, attitudes toward medicine, and even the outcome of their medical treatments are all influenced by the cultures that comprise them. The cultural and ethnic diversity of the HIV/AIDS epidemic in the United States is well known. The vast majority of people living with HIV/AIDS (PLWHA) today are members of historically underserved and marginalized populations, most notably racial and ethnic minorities. This four-session learning collaborative explored cultural humility and showed how to reduce stigma and discrimination.

Impacts of Housing Instability on the Health of Vulnerable Populations

Research increasingly shows the deep impact that housing instability has on the physical and mental health of our patients. Scholarship has indicated the primary role that promoting housing stability has on improving outcomes in vulnerable communities and the unique strategies that institutions and providers can use to network individuals to housing opportunities as well as to wholistically support the unhoused, HUD-assisted families and residents of public housing.

In this webinar, Dr. Kevin Lombardi MD, MPH, the NCHPH team and colleagues presented a unique perspective to supporting these vulnerable populations. Through the lens of the Social Determinants of Health (SDOH) we engaged with case studies, promising practices and the latest research and clinical recommendations to provide a wholistic perspective of how providers and institutions can promote the health and wellbeing of the unhoused, HUD-assisted families and residents of public housing.

Among the interventions covered were how institutions can utilize Community Health Workers (CHWs), inter-organizational partnerships and trauma-informed care to improve their quality of care.

Colorectal cancer (CRC) is a leading cause of cancer-related deaths in the United States, despite the availability of high-quality screening options. Multiple barriers to CRC screening have been identified, including cost for the uninsured, younger age, racial or ethnic minorities, recent immigration to the United States (non-English speakers), fear or distrust of the health care system, and rural residence. This learning collaborative will explore barriers to CRC screening and identify interventions that can be implemented by health center programs serving at-risk populations.

Previous research has indicated that individuals with physical or intellectual disabilities often struggle to access the resources and care necessary to live independent and healthy lives. Scholarship has also indicated that providers and supporters of these individuals often face frustration and burnout when they are unable to provide patients with the support they need.

In this 4-part learning collaborative, Dr. Kevin Lombardi MD MPH and the NCHPH research and clinical quality team facilitated a set of interdisciplinary and collective sessions designed to engage with the latest data, clinical/social guidance as well as recommendations from leading providers on how to maximally support individuals with disabilities.

Leveraging Telehealth for use in Tobacco and Nicotine Cessation

Recent studies have indicated that telehealth can serve as a critical tool in providing nicotine and tobacco-use cessation counseling. However, organizations often struggle to promote, market and provide these resources in an efficient manner. Fortunately, there are frameworks, research and protocols available that can help us to leverage and promote our telehealth resources while maximally supporting our patients and taking full advantage of staff resources. According to the Centers for Disease Control and Prevention, up to 1 in 4 adults in the United States have some type of disability, and 1 in 5 of them did not have a routine check-up in the last year. Disability inclusion allows for people with disabilities to take advantage of the benefits of the same health promotion and prevention activities experienced by people who do not have a disability. Challenges to community integration for people with disabilities also need to be addressed through innovative integrated care programs in the community. This webinar explored health centers’ and public housing authorities’ initiatives to improve access to primary care and addressed social determinants of health affecting people with disabilities.

Screening for Colorectal Cancer: A Patient Centered Approach to Improving Marketing and Promotion

Screening is an essential tool in the prevention of colorectal cancer. Nearly 2 million people in the U.S. are diagnosed with colorectal cancer per year, a significant proportion of these cases could be prevented or diagnosed sooner in screening was more widely utilized by the public.

In this webinar, Dr. Kevin Lombardi and the NCHPH team took a practical, epidemiological view of the promotion and marketing of colorectal screening services. The session included a review of validated promising practices, screening tools and marketing procedures that have been shown to increase rates of colorectal screening in populations of FQHC patients.

Special attention was given to the cultural, linguistic and social conditions that increase screening use in patients from traditionally medically underserved communities and the clinical and workforce management processes that lead to more patient-centered care. The session format included case studies and discussion portions designed to illicit a meaningful, enjoyable and collective interdisciplinary learning experience.

Hosted by the National Nurse-Led Care Consortium and the National Center for Health in Public Housing, this 2-part webinar series discussed promising practices in home visitation and telehealth as ways to support improved access to comprehensive primary care for communities with high levels of disability, and isolation, lack of adequate transportation, and other social drivers of health that contribute to health inequities, particularly residents of public housing.

In the first session, we examined how home visitation can be integrated into the provider panel to expand access for aging and disabled patients residing in public housing. In the second session we addressed the benefits of telehealth, recent changes to utilization, and how to implement telehealth services to expand access in health centers.

Ready to Respond: Behavioral Health and Addiction Management Beyond the COVID-19 Pandemic

The COVID-19 pandemic had a profound impact on the frequency of addiction and behavioral health conditions observed by FQHC’s and PHPC’s. Moreover, many aspects of the COVID-19 response and recovery complicated the treatment of existing patients and increased the number of individuals seeking care.

In the webinar, the National Center for Health in Public Housing team looked at the models and practices used by health centers to manage patients and organizations in that complex environment. The content focused on promising practices, workflow management, and recent industry advancements.

In this three-part webinar series hosted by the National Nurse-led Care Consortium (NNCC) and the National Center for Health in Public Housing (NCHPH), viewers can expect to learn about improving maternal and child health equity with home visiting program partnerships and health centers. This series explores opportunities for health centers to develop partnerships with home visiting programs shown to improve maternal and child health outcomes, promote health equity, and reduce health disparities.

The first session covered trends in the current landscape of Maternal, Infant, and Early Childhood Home Visiting (MIECHV) funded programs and where existing and future partnerships with health centers are possible. In session two, participants heard about community health centers’ return on investment (ROI) and increased patient health outcomes after home visiting integration. And the third session showcased strategies for creating action and sustainability plans to implement a home visiting partnership for interested health centers.

The Challenges in the Development of Diabetes Prevention in Public Housing Settings

Prediabetes is a serious health condition where blood sugar levels are higher than normal, but not high enough yet to be diagnosed as type 2 diabetes. Approximately 96 million American adults—more than 1 in 3—have prediabetes. Of those with prediabetes, more than 80% don’t know they have it. Prediabetes puts people at increased risk of developing type 2 diabetes, heart disease, and stroke. Patients with prediabetes living in public housing communities face unique challenges related to lack of awareness, difficulty in accessing health care systems and medications, and consequently failure in achieving optimal diabetes management and preventing complications. This webinar explored the challenges in the development of diabetes prevention in public housing settings and interventions to address these challenges.

Misinformation and disinformation related to COVID-19 and electronic nicotine delivery systems (ENDS) are increasing. Some of this may stem from early reports suggesting a lower risk of severe COVID-19 in nicotine users.
