Chronic diseases impact one of every two adults and account for 86% of our nation’s healthcare spending. These conditions also have a disproportionate impact on traditionally marginalized communities including racial and ethnic minorities, LGTBQIA+ community members, recipients of HUD funding and Residents of Public Housing. Fortunately, many chronic diseases such as cardiovascular disease and type-two diabetes mellitus are preventable and patients can now live long and healthy lives when provided with the appropriate supports.
In this webinar, Dr. Kevin Lombardi MD, MPH and the NCHPH Research and Clinical Quality teams presented a comprehensive overview of the latest clinical, programmatic and social guidance for chronic disease prevention, treatment and support. Covered chronic diseases included cardiovascular disease, type-two diabetes mellitus, COPD and conditions of chronic pain with consideration also given to HIV and Rheumatological conditions.
