This webinar by NCHPH will highlight the importance of housing as a social determinant of health by describing the health care needs and challenges of individuals that are homeless, live in supportive housing, or are public housing residents. Panelists reported and compared health care utilization and prevalence of diseases for these vulnerable populations and offered best practices on ways to improve access to care.

Background:

This webinar highlights the importance of housing as a social determinant of health by describing the health care needs and challenges of individuals that are homeless, live in supportive housing, or are public housing residents. In this webinar, panelists report and compare health care utilization and prevalence of diseases for these vulnerable populations and offer best practices on ways to improve access to care.  This activity is part of an ongoing effort to provide training and technical assistance on issues related to housing and health.

Learning Objectives:

  • Participants will recognize the unique health care needs of patients experiencing homeless, living in supportive housing, or are public housing residents.
  • Participants will be able to describe data supporting the health needs of health center patients in various housing situations.
  • Participants will be able to describe strategies and identify opportunities to apply them to improve access to care for patients experiencing homeless, living in supportive housing, or are public housing residents.
