Summary:

The National Center for Health in Public Housing is pleased to present “What is Health Data Telling you? Using HIE Data for Public Health and Health Promotion Interventions.” As Health Information Exchanges become more robust in the types and amount of data they are receiving from various healthcare stakeholders, leveraging aggregated data from HIEs, as well as the community, is proving valuable in population health management efforts. This session will describe how Maryland Department of Health and Mental Hygiene is utilizing HIE data for public health analysis, and how a Public Housing Primary Care Health Center participated in a pilot HIE program in the city of San Diego, California.

 

Objectives:

  • Explain how MD Department of Health and Mental Hygiene (DHMH) and Chesapeake Regional Information System for our Patients (CRISP) work together and utilize HIE data for public health analysis
  • Discuss how DHMH is taking public health action based on HIE data
  • Describe La Maestra Community Health Centers’ HIE Experience and outline health promotion priorities
