81.4% of PHPC health centers who responded to the 2021 National Training and Technical Assistance Needs Assessment survey responded that they need TTA on strategies and tactics for addressing community-level barriers to health equities. NCHPH has developed T/TA that focuses on increasing the number of health centers that understand the importance of SDOH using strategies that include:
1. Providing T/TA on the importance of screening for social factors and how each can impact health outcomes
2. Educating health center staff on the relationship between social determinants of health and health disparities
3. Educating health center staff on identifying and addressing social determinants of health during clinical encounters to improve outcomes and health equity.
