This four-session Community of Practice examined prevention and practice approaches that support individuals with disabilities experiencing behavioral health conditions. Participants explored disability-informed strategies that strengthen screening processes, improve treatment engagement, and enhance continuity of care within health center settings. The series reviewed epidemiologic trends, risk and protective factors, and simulated primary care scenarios to demonstrate how clinical workflows, care coordination models, and structured non-clinical supports can improve prevention efforts and treatment engagement. Presenters shared practical implementation insights that health centers can apply to strengthen service delivery for individuals with physical, intellectual, and developmental disabilities.
