Managing diabetes effectively through self-care behaviors can be particularly challenging for health center patients with physical, sensory, intellectual, and developmental disabilities. Individuals with disabilities experience higher rates of diagnosed diabetes and often face additional barriers related to mobility limitations, communication differences, cognitive functioning, caregiver dependence, and access to coordinated services. This webinar will explored the unique obstacles that may hinder effective diabetes self-management and provide practical strategies health centers can implement to strengthen care coordination and patient engagement. The session reviewed workflow adaptations, caregiver education approaches, and community collaboration strategies that support continuity of care. Participants learned how to identify patients’ special needs and housing status, reinforce individualized self-care practices, and initiate collaboration with public housing groups to improve chronic disease management outcomes.
