People living with HIV often have health-related needs that extend beyond clinical care. Factors such as housing stability, substance use, food access, transportation, and social connection can affect engagement in care, medication adherence, and long-term viral suppression. Community-based interventions, including those supported by Community Health Workers (CHWs), help patients navigate health care systems and connect to community resources. Through trusted relationships and tailored support, CHWs reinforce care plans and address barriers that may interfere with ongoing treatment. This webinar examined community intervention models that support people living with HIV, with a focus on individuals in public and affordable housing. Presenters discussed how health centers can deploy CHWs to address health-related needs, strengthen care coordination, and support retention in HIV care. Practical examples and implementation insights highlighted real-world workflows, role clarity, and documentation practices that support sustainable HIV care programs.
